A hospital digital marketing audit is the cheapest bargaining power you will ever hold over an agency, and most hospital owners never use it. It takes an afternoon. It needs no technical degree. And it can change the terms of an agency contract that renews in thirty days.

Here is how the renewal usually goes. An email arrives. It is friendly. It mentions a “great quarter,” attaches a report full of impressions, and asks you to sign for another year. You open the report. The graphs climb up and to the right. You still cannot say how many patients walked through the door because of any of it. Most owners sign anyway, because checking feels like accusing someone. It is not. It is housekeeping, the same kind you would do on any large expense.

That gap, between what the report shows and what you can actually point to, is exactly what a hospital digital marketing audit closes. Below you will find a seven-check version you can run yourself, a plain way to score what you find, and what to say in the renewal meeting once you have it. Whether you end up renewing, renegotiating or walking away, you will decide with evidence instead of a feeling in your stomach.

What a hospital digital marketing audit shows that monthly reports never will

Monthly reports are written by the people being judged. That is not an accusation. It is how the setup works. An agency chooses which numbers go on the page, and it may lean toward the ones that look best: impressions, clicks, followers, keyword rankings. All real. None of them pays a hospital’s bills.

A hospital digital marketing audit turns the question around. Instead of asking what the agency did this month, it asks what is actually true about your marketing today. Who owns the accounts. Whether calls are counted. What kind of enquiries arrive. Whether money is leaking somewhere in the background.

Think of the report as the agency’s account of the year, and the audit as the medical record. Both matter. Only one was written by someone with no stake in how it reads.

The one question that changes every renewal meeting

Walk in with a single question: how many patients came from this, and how do you know?

Then watch. A good agency answers with numbers, a method and a few honest caveats. A weaker one answers with traffic. Sessions are up. Rankings improved. Social reach doubled. Everything except the thing you asked.

The question is harder than it sounds, and it is only fair to say why. Patient journeys can stretch from 8 to 180 days, which makes last-click attribution wrong far more often than right. On top of that, many hospitals record form submissions in Analytics while phone enquiries go uncounted, so digital channels can look stronger than they are.

So no agency can give you a perfect answer. But a good one gives an honest, structured one, and that difference is what a hospital digital marketing audit measures. Everything below is built to help you see it.

The quiet cost of never checking

Nobody decides to drift. It happens by increments. The agency sends a report each month, you skim it, nothing looks alarming, and the contract rolls forward. A year later you are paying the same fee for the same activity, with no clearer idea of what it produces.

Small leaks add up in a way that never shows on a single invoice. A tracking tag that broke in March and was never fixed. A campaign still aimed at a city you stopped serving. A Google listing with a wrong phone number that sends callers to a dead line. Each one looks minor. Together they can swallow a good share of your budget as wasted ad spend, while the marketing reports keep saying everything is fine.

A hospital digital marketing audit is how you catch those leaks while they are still small. It does not require anyone to have done wrong. It only requires someone to look.

The seven-check hospital digital marketing audit you can run in an afternoon

Gather five things first: the signed agency contract, the last three marketing reports, login details for every account (including whichever platform runs your call tracking), a list of your top specialties by revenue, and a quiet room with a phone. That last one matters. You will be listening to call recordings, and it is hard to judge a patient’s tone while someone talks in the next cubicle.

Set aside three hours. Open a notebook, or a spreadsheet if you prefer. Give each check in your hospital digital marketing audit one point if it passes and zero if it does not. No partial credit. Partial credit is where weak results like to hide.

Seven-point hospital digital marketing audit covering account ownership, lead tracking, SEO, ad spend, reporting and agency contracts.

1. Do you actually own your accounts?

Log in to Google Ads, Meta Ads, Analytics, Search Console and your Google Business Profile. Is the owner email yours or the agency’s? If you cannot get in, or everything sits under the agency’s name, the rest of the audit gets harder and your bargaining power shrinks. An agency that makes it difficult to reach your own Analytics and Search Console data is a significant concern.

2. Is every enquiry being counted?

Check call tracking, form tracking and WhatsApp tracking. Place a test call from your own phone. Submit a form. Tap the WhatsApp button. Then look for all three in the data. If the calls never show up, your reports are describing only part of the picture, and probably the flattering part.

3. What is the lead quality?

Ask for the last twenty enquiries, with their source. Listen to the call recordings or read the messages. How many came from the right city, for the right specialty, with a real intention to book? Cheap enquiries from the wrong place are not cheap. A table of numbers cannot show lead quality, but twenty phone calls can. For a deeper look at what good enquiries look like, see our guide to patient lead generation for hospitals. And remember that enquiries often die in the gap between a booked appointment and an attended visit, a gap most agency reports never mention. India-specific detail on this sits in upGrowth’s patient acquisition playbook.

4. Is the SEO foundation healthy?

Search your three biggest specialties plus your city. Where do you appear? Does each specialty have its own page, and does each doctor? Does the site load fast on a phone? Then search your hospital name and look at the Google Business Profile: hours, phone number, categories, photos, and whether reviews, including the angry ones, get answered. A professional SEO review goes further, covering crawlability, indexation, schema, competitors and citation consistency, but you can spot the obvious gaps yourself. If SEO is where most of your fee goes, our guide to evaluating a hospital SEO agency shows what good work looks like.

5. Is any money being wasted?

Open the search terms report in Google Ads. Which phrases triggered your ads? If you see job seekers, other cities, “free” searches or students, you have found wasted ad spend. Check location settings and negative keywords too. Wasted ad spend is rarely dramatic. It is a steady drip, and the agency’s fee does not change while it drips.

6. Do the marketing reports show outcomes?

Take the last three marketing reports and sort every metric into two piles. Activity: impressions, clicks, rankings. Outcomes: qualified enquiries, booked appointments, cost per attended patient. A reporting deck built mainly on rankings and impressions is a recognised red flag, while cost per booked appointment and show rate are among the metrics that actually predict bookings. If the outcome pile is empty, you have your answer. For the maths behind these metrics, Zocdoc’s guide to healthcare marketing ROI is a useful companion.

7. Was the agency contract delivered?

Put the agency contract beside the actual work. It promised twelve service pages: how many exist? Monthly blogs: where are they? A tracking setup: is it live? Tick off each promise. Gaps are not always bad faith. Sometimes work simply slid down the priority list. But anything you paid for and did not receive is renewal bargaining power, plain and simple.

Renew, renegotiate or walk away

Add up the points from your hospital digital marketing audit.

  • 6 or 7: The agency is doing real work. Renew, tighten the KPIs, and use your audit to ask for small improvements.
  • 3 to 5: Renegotiate. Do not sign the same agency contract again. Ask for a 30-day fix list, a shorter term and outcome-based reporting.
  • 0 to 2: Replace, or at least invite a competing pitch. Begin planning the handover while your notice rights still exist.

These bands are a guide, not a law, and every hospital digital marketing audit should be read in context. A failed check 1 deserves extra weight, because without your own accounts you cannot leave cleanly. Look for auto-renewal clauses too. Some contracts roll over unless you write in time, and the date for that is often earlier than people expect.

The fix list that comes out of a hospital digital marketing audit works best when it is specific. For example:

  • Move the Google Ads and Analytics accounts into the hospital’s name by day 7
  • Repair call tracking and test it with five live calls by day 14
  • Remove irrelevant search terms and add negative keywords by day 21
  • Deliver a new report showing booked appointments by day 30

Dates matter more than promises. A fix without a deadline is a wish.

How to take your hospital digital marketing audit into the renewal meeting

Bring a one-page summary: the seven checks, your score, and the three biggest gaps. Share it before the meeting if you can, so nobody feels ambushed. Then ask for three things.

  • A dated fix list for every gap
  • Reports that show enquiries, booked appointments and cost per attended patient, by service line
  • Ownership of every account confirmed in writing

Pay attention to tone. A good agency treats a hospital digital marketing audit as proof that you are serious, and it responds with answers. A defensive one responds with a longer contract. If the answer to every request is more months on the dotted line, you have learned what you needed to learn.

At Digital Chaabi, we welcome this kind of scrutiny. Our own work with hospitals starts with an audit and tracking before any campaign goes live, so the client holds a baseline to measure us against. Nobody should have to take an agency’s word for results.

What if the agency pushes back?

Some will. It helps to know the three most common replies.

“You cannot measure patients, so the audit is unfair.” Partly true, as we saw earlier. But a hospital digital marketing audit never demands perfect attribution. It asks whether calls, forms and WhatsApp are counted, and whether the agency tries to follow an enquiry toward a booking.

“You do not understand SEO, it takes time.” Also partly true. SEO is slow. But slow does not mean invisible. Even by month three, you should see technical fixes, new pages and a clear plan.

“Our other clients are happy.” Maybe they are. Ask to speak to one. A confident agency will say yes before you finish the sentence.

Pushback is information. Watch whether it arrives with answers or with irritation. At Digital Chaabi, we would rather be asked these questions than dodge them.

When you would rather not do it alone

Seven checks in an afternoon is realistic. But teams are busy, and sometimes the agency will not share access at all. In those cases an outside hospital digital marketing audit is cleaner: fresh eyes, no politics, and a written report you can lay on the table. Digital Chaabi runs these both before renewal and in the middle of a contract. Even if you stay with your current agency, you leave with a clear list of what to fix.

And keep an eye on the clock. Every month on an auto-renewing contract, without an audit, is another month of paying for results nobody has verified.

Before you sign anything

Think back to that friendly renewal email. It reads differently now, does it not? The “great quarter” is a claim waiting to be checked, and you know how to check it.

A hospital digital marketing audit will not make you suspicious of your agency. It makes you informed, which is a calmer place to negotiate from. Good agencies pass the audit and earn their renewal. Weak ones get a clear list of fixes, or a polite goodbye.

Renewal is the one moment in the year when you hold the pen. Use it before somebody else does.

Frequently Asked Questions

How to conduct a digital marketing audit?

Work through these in order:

  • Account access and tracking
  • Lead quality, SEO and paid campaigns
  • Reports checked against the contract

For hospitals, a hospital digital marketing audit should also include listening to real enquiry calls.

How to promote a hospital in digital marketing?

Start with a strong Google Business Profile and one page per specialty, then add search ads for high-intent queries and track every call and form. Reviews and patient education content build trust over time. A hospital digital marketing audit shows which of these is actually working.

What are the 5 P’s of healthcare marketing?

Commonly they are product (your services), price, place (how patients reach you), promotion and people (doctors and staff). Sources vary slightly on the fifth P. A hospital digital marketing audit checks how clearly each one shows up online.

What are the 7 Ps of service marketing in hospitals?

The five above, plus process (how booking and care flow) and physical evidence (facilities, reviews, website). Process and physical evidence are where enquiries often leak, so a hospital digital marketing audit looks hard at both.

How often should a hospital audit its marketing?

  • Yearly, as a full review
  • 60 days before any agency contract renews
  • After big changes, such as a new branch or website

Quick monthly checks on tracking keep surprises small.

What should an audit report show before renewing an agency?

Scores for each check, the three biggest gaps, evidence such as call samples and screenshots, and a dated fix list. It should say plainly whether results justify the current agency contract, so a hospital digital marketing audit ends in a decision, not a pile of charts.
